Jason Richard Hopkins,  49,  Salem died Sept. 29, 2021, in Baptist Health Floyd New Albany.

Born Oct. 31, 1971, in Louisville, he was the son of Richard I. and Sandy K. (Lashbrooks) Hopkins.

There will be a graveside service Saturday, Oct. 23,  at Crown Hill Cemetery at 10 a.m.

Dawalt Funeral Home was entrusted to arrangements.
